Foster CareCSG Chariots program provides a safe, nurturing environment for children and families in need of out-of-home therapeutic care. Children ranging from infants to age 21 are offered intensive and comprehensive care that meets their individual needs and provides support to the child, natural family, and treatment family. We offer a network of services to assist the child and family in reaching their goals. As a result, children have the opportunity to develop trusting, stable relationships with our team of professionals, provider parents, and their family. Our programs consist of:
Community-Based ServicesCSG is dedicated to improving the lives of children with mental health conditions by providing a full range of services available in the home, community or school setting. Community-based programs include: Intensive Case ManagementThe Intensive Case Management Program (ICMP) provides services to adults, children and adolescents living with serious and persistent mental health diagnoses. ICMP services impact all areas of an individual’s life. Services include coordinating, linking, monitoring, accessing community services and building a natural support network. ICMP is individual driven and community based, occurring at the individual’s home, work site or at other designated locations convenient for the individual. Behavioral Health Rehabilitation Services (BHRS)BHRS program is designed to enhance the scope of community-based services for children, adolescents and their families. Goals focus on interventions that support the at-risk child or adolescent’s continued participation in the community, thereby preventing a placement into a more restrictive setting. BHRS services are designed to address the needs of children and adolescents. An interdisciplinary team determines duration and frequency of services. Services are also based on documented assessment, diagnosis, and projected outcome. Services are also used to increase the transition of children or adolescents returning to the community from an out-of-home placement. Family Based Mental Health ServicesThe Family Based Mental Health Services (FBMHS) Program is an intensive service provided in the family home and community for children and adolescents up to the age of 21 in jeopardy of being hospitalized or referred to out-of-home placements due to a mental health diagnosis or family challenges. |
