CASE Management

Case Management provides support and care coordination to individuals with complex needs and services systems. Case managers work with each person in their home and in community settings to provide support when and where it is needed. Services include coordinating, linking, monitoring, accessing community services, recovery and resiliency planning and building a natural support network.
